2. Hotel Accommodations
Nippon Travel Agency has reserved rooms in Tokyo during the congress period. Reservation will be made on a first-come, first-serve basis. Room rates are listed in Table 2.

3. Application and Payment
Those participants who would like to book hotel room(s) described herein are kindly requested to complete the Application Form and return it to Nippon Travel Agency no later than October 12, 2003. Upon receipt of the Application Form, Nippon Travel Agency will send you Confirmation & Invoice with total amount due. All payments must be in Japanese Yen. If you choose payment by credit card, the total amount will be charged to your credit card prior to the symposium. Handling charge 500 Yen per one booking is added to hotel fee. Thank you for your acceptance in advance.

4. Cancellations
In the event of cancellation, written notification should be sent to Nippon Travel Agency. The following cancellation fee will be deducted before refunding.
- 13 to 4 days before first night of stay:   30% of daily room rate per room.
- 3 days to 1 day before first night of stay:   50% of daily room rate per room.
- On the day of reservation or no notice given:   100% of daily room rate per room.
